Class Member Forms

October 14, 2004 is the Last Day to File Proof of Claims.
There are two steps for class members to follow in seeking recovery from the settlement fund.
The first step is to file a claim form which must be done before you are eligible to proceed to the next step. The claim form is simple and can easily be filled out either on line by selecting the link to the "Web Claim Form", and filling it out online on or before May 21, 2004 or it can be printed out by selecting link to the "Adobe Acrobat Claim Form", printing this form, completing it and then mailing it to the court at the required address, to be received, on or before May 28, 2004.
The second step requires all those who have filed claim forms to submit proof of those claims to the court. This step is more complicated and you may wish to seek the assistance of counsel in proving your claim although you are not required to do so. The procedures to be followed in proving your claim are set forth below and can be accessed by selecting the link "Proof of Claims Procedures".

You must complete a Claim Form to be eligible for recovery from the settlement fund.  You may contact us if you would like assistance in filling out the claims form.  Call us at 1-800-348-3805 and tell the receptionist that this call relates to Exxon claims.  You may also email us at exxonca@speightsrunyan.com.

  • ADOBE ACROBAT CLAIM FORM: *** THE FINAL DATE FOR ONLINE CLAIM FORM SUBMISSIONS HAS PASSED.  YOU MUST NOW FILL OUT THIS ADOBE ACROBAT CLAIM FORM AND DELIVER IT TO THE ORANGEBURG COUNTY COURTHOUSE.  CLAIM FORMS MUST BE SUBMITTED AT THE COURTHOUSE NO LATER THAN FRIDAY, MAY 28TH.

Proof of Claims Procedures
Proofs of all claims must be filed with the Orangeburg County Clerk's Office, in person or by mail, by August 30, 2004.
